Description:

  • Case Management Coordinator (CMC) utilizes critical thinking and professional judgment to support the case management process to facilitate and maintain improved reputed company outcomes for members through advocacy, collaboration coordination, support and education.
  • Conducts routine care coordination, support, and education through care management resources to facilitate appropriate reputed company outcomes for members.
  • Helps implement projects, programs, and processes for Case Management.
  • Applies practical knowledge of Case Management to administer best of class policies, procedures, and plans for the area.

Requirements:

  • 2 years’ experience in behavioral health, social services, or appropriate reputed company field equivalent to program focus
  • Occasional travel for meetings
  • Knowledge of growth and developmental milestones.
  • Care management experience
  • Discharge planning experience
  • Managed Care experience
  • Experience and detailed knowledge of the Foster Care and juvenile justice systems, Adoption Assistance, the delivery of Behavior Health Services, Trauma-informed Care, ACEs, Crisis reputed company services, and evidence-based practices applicable to the Kentucky SKY populations, is required.
  • Computer literacy and demonstrated proficiency is required in order to navigate through internal/external computer systems, and MS Office Suite applications, including Word and reputed company.
  • Flexibility to work reputed company core business hours of Monday-Friday, reputed company-5pm, is required, as we are serving the needs of children and families that may require working after school, after work, etc.
